A Guide to Pulsed Lavage

When treating wounds they now use pulsed lavage. Pulsed lavage or pulsatile jet lavage is a kind of mechanical hydrotherapy which makes use of a solution which is pressurized and pulsed so that the wound can be irrigated and debrided. In a lot of cases, they use suction with pulsed lavage. The reason for this is so that both wound debris and the irrigation solution can be removed. When the wound bed is debrided of devitalized tissue, pulsed lavage seems to improve the growth of granulation tissue. And this is done without disrupting the normal tissues that lie beneath it. Granulation is also stimulated with the creation of negative pressure by the suction.

Here are some important tips that clinicians should keep in mind when they perform pulsed lavage.

First is that you can adjust the force of pressure used during pulsed lavage. This should be regulated according to the amount of slough or necrotic tissues that require debridement and it should also be regulated according to the tolerance of the patient. You can set it to 4 to 12 psi and to obtain a greater amount of debridement, you can increase the psi. If the patient cannot tolerate it, then you should decrease the psi to lessen the pain.

If during the procedure, the patient continues to feel pain, then you can use several additional modalities. You can premediate the wound bed with a topical analgesic solution. In order to give added relief, you can premedicate the patient 30 minutes before the procedure with the appropriate medication for pain.

You can put antibiotic to the normal saline solution if there are signs of infection with devitalized tissue. With antibiotics, the bioburden on the surface of the wound is reduced and will help remove the infected tissues.

Pulsed lavage can be used in many patient setting because of its portability and relatively low cost. It can e used at the bedside in acute care hospitals, long-term care facilities, and outpatient physical therapy settings.

There is one concern, though, about pulsed lavage and that is the physiological cooling of the wound bed. When this happens, the healing process of the wound can slow down. This can be remedied by warming the bed of saline solution to room temperature to minimize the effect.

With pulsed lavage, operating rooms are freed up, safety is increased, contamination reduced and creates new hope for permanent wound healing.

And the discomfort that the patient experiences when undergoing debridement is very minimal and they can well tolerate it. If this is used for daily treatment, then the wound bed preparation is enhanced for skin graft or for normal healing.
